I participated in a one-month practicum in June 2019 at Northwest Hospital and Medical Center, one of the University of Washington's affiliated hospitals. Before traveling, I took an online educational program and, as a 'Pharmacy Assistant' (a qualification that allows one to perform limited duties under the supervision of a local pharmacist), I worked alongside a pharmacist dedicated to the emergency ward. My main duties included determining the dosage of high-risk drugs such as antimicrobials and anticoagulants, as well as the types and ratios of ingredients in nutritional supplements. In the United States, laws and procedures vary by state, so it was necessary to prepare according to the requirements of the practicum site. However, with the support of those around me, the preparation itself was also a good experience.

In Japan, regarding the dosage and administration of drugs, pharmacists can only make 'proposals' to doctors; they do not have the final decision-making authority. In the United States, pharmacists have significantly more decision-making power compared to Japan, and the dosage and administration I designed were directly applied to the patient's treatment. The responsibility is great, but it was truly rewarding to see patients' conditions improve, and I strongly felt a sense of unity working as a 'team' with doctors and other professionals.   

The practicum ran from 7:00 a.m. to 3:00 p.m. on weekdays. After the practicum and on weekends, I enjoyed the city of Seattle with my host family and local students. My host family was especially kind, and we still keep in touch through email. It was a wonderful encounter.

Before traveling, I was unsure if I could handle the language and the content of the practicum, and I felt anxious. However, being able to participate in a program I had long admired was a truly valuable experience. If you are hesitating, I encourage you to take on the challenge!
