先天性心・大血管奇形(CHD)を合併した新生児37例について診療録を後方視的に検討した。疾患の内訳は食道閉鎖症(16.2%)、横隔膜ヘルニア(16.2%)、鎖肛(13.5%)が多く、腹壁異常(8.1%)や気管・気管支異常(8.1%)が続いた。代表的な小児外科疾患のCHD合併率についてみると、食道閉鎖症、直腸肛門奇形、気管支系異常で合併率が高かった。死亡率は24.3%(9/37例)で、小児外科で治療を行った新生児症例全体(408例)の死亡率12.5%に対し約2倍と高値を示した。疾患別にみると、CHD合併の食道閉鎖症や直腸肛門奇形では比較的死亡率が高かったが、先天性横隔膜ヘルニアではCHD合併と非合併との間で生存率に差はなかった。また、出生前診断の有無でみると、診断の有無で生存率に差はなかった。CHDを高率に合併する食道閉鎖症および気管支系疾患について治療戦略を検討した結果、生直後に生命維持に直結する小児外科的奇形の修復を行い、次いで姑息手術などにより循環機能の安定を図り、その上で循環機能の予備力を評価しつつ外科疾患とCHDの根治的手術を行うべきであることが示された。

画像検査でリンパ管腫と診断された患者9名12病変(男児3名、女児6名、7ヵ月〜5年7ヵ月)を対象に、リンパ管シンチグラフィーを行った。トレーサー注入部位は頬部4、下顎下、側胸部各2、鎖骨上、下顎部、肩甲骨上、脇窩各1で、嚢胞内7部位(I群)、嚢胞外5部位(E群)であった。左側側胸部皮下の嚢胞内注入例では、嚢胞部は4時間後も強いradioactivityを示し、病変内でトレーサーの拡散を認めたが、流出を示す線状の集積はなく、体腔臓器に僅かな集積を認めた。24時間後は局所のトレーサーは更に広がったが、範囲は体表での確認病変と一致していた。対照の正常部位皮下では注入後速やかに体中枢への線状集積を認め、頭部皮下注入でも鎖骨上窩への線状集積を認めた。放射線集積残存率は、注入早期(1〜5時間)/後期(17〜24時間)ではリンパ管腫群92.1%/58.1%、対照群61.0%/14.0%と両期とも有意差を認め、I群とE群では両期とも有意差は認めなかった。

症例は16歳女性,咳嗽,発熱,左前胸部痛を主訴に来院.胸部CTにて内部不均一な径7cmの前縦隔腫瘍及び左舌区・下葉の完全無気肺を確認,また,MRIにて前縦隔腫瘍内に脂肪組織と同一の吸収域を認め,成熟型奇形腫穿破による膿胸と診断.膿胸に対し胸腔内繊維素溶解療法,ドレナージ及び抗生剤にて加療,膿胸改善を確認した上で,前縦隔腫瘍摘出術施行,病理にて嚢胞性成熟型奇形腫と診断した.病理組織にて膵類似の腺組織を確認,穿破の原因として,腫瘍内膵酵素の存在が考えられた.成熟型縦隔奇形腫は穿破により重篤な合併症発生の危険性があり,また経過中悪性転化する可能性もあることから,早期の外科治療が重要と考えられた.(著者抄録)

To determine the most appropriate treatment strategy for neonates with coexisting congenital heart diseases (CHD) requiring surgery, 408 neonates treated in our department between 2002 and 2009 were retrospectively reviewed. Of the 408 neonates, 37 (9.1%) had coexisting CHD. Among the pediatric diseases requiring surgery, esophageal atresia (46.7%), anorectal malformation (24.1%), and tracheobronchial anomalies (75.0%) were frequently associated with CHD, and the survival rate among patients with coexisting CDH was 24.3%, about 2-fold higher than among those without coexisting CHD. Clinical observations in the present series suggested that fatal conditions such as tracheoesophageal fistula should be repaired immediately after birth, followed by palliative cardiac surgery to stabilize the circulatory status if necessary. Thereafter, radical repair of cardiac and pediatric anomalies should be performed, taking into account the balance between surgical stress and cardiac reserve. For this purpose, proper assessment of surgical stress and cardiac function and collaboration among specialists in relevant fields are essential during the perinatal period to determine the treatment strategy for patients with multiple-organ anomalies.

A 16-year-old female visited our hospital, complaining of cough, fever, and left precordial pain. Chest computed tomography showed a heterogeneous anterior mediastinal tumor measuring 7 cm in diameter and complete atelectasis in the lingula and lower lobe of the left lung. Magnetic resonance imaging also showed an area of intensity identical to that of adipose tissue in the anterior mediastinal tumor. Thus, empyema due to rupture of a mature teratoma was diagnosed. The empyema was treated with intra-pleural fibrinolytic therapy, drainage, and antibiotics. After confirming resolution of the empyema, we resected the anterior mediastinal tumor and pathologically diagnosed it as cystic mature teratoma. Histopathological examination showed glandular tissue resembling the pancreas, suggesting that the rupture had been caused by pancreatic enzymes in the tumor. Mediastinal mature teratoma carries a risk of serious complications developing due to rupture and the possibility of malignant transformation during the disease course. Thus, early surgical treatment is important.

胸膜肺芽腫6例(男児2例・女児4例・2歳6ヵ月〜3歳2ヵ月)の治療成績を報告した。1970年後半から1980年後半の時期に経験した3例のうち、1例は緊急手術で亜全摘となり、術後VAC療法と放射線治療を行ったが、再燃して6ヵ月後に死亡した。病理診断はtype IIIであった。他の2例は術前にVAC療法と放射線治療を行い、腫瘍は縮小して全摘出でき、病理診断はtype II、IIIであった。術後もVAC療法および放射線治療を行い、治療終了後それぞれ約23年、25年経過し再発はない。2004年以降経験した3例では、new A1プロトコールにvincristine(VCR)を加えた術前化学療法で腫瘍縮小を得た2例を含め、いずれも全摘できた。病理診断はtype IIが2例、IIIが1例で、術後はnew A1+VCRを全例に、CPM+VCR+VP16+CDDPを1例に、大量化学療法+末梢血幹細胞移植と放射線治療を1例に行った。これら3例は、治療終了後1〜6年経過し再発なく生存中である。

Hepatocellular carcinoma (HCC) in children is rare, and the prognosis has been poor because of its advanced stage at diagnosis and unresponsiveness to chemotherapy. We report a 13-year-old boy with ruptured HCC in the left trisegment. When hemostasis of the ruptured surface was achieved in the emergency operation, the left branch of the portal vein and the left hepatic artery were ligated at the same time. The volume of the future liver remnant (FLR), that is, his right posterior sector, increased from 56% on admission to 70% of his standard liver volume on day 2. Blood level of serum protein induced by vitamin K absence or antagonist II started to decrease immediately. Left trisegmentectomy was successfully performed 10 days later, followed by chemotherapy. He has been well with a 2-year survival without recurrence. When the FLR is considered relatively small for a major hepatic resection, the selective ligation of the portal vein and the hepatic artery, which feed HCC, seems to be beneficial. This is because it may induce enlargement of the FLR, increasing the safety of the hepatectomy as preoperative portal vein embolization does before a major hepatectomy in adult patients with HCC, and the latter suppresses the tumor while waiting for the planned hepatectomy. © 2011 Elsevier Inc.

serial transverse enteroplasty(STEP)手術を行った短腸症候群3症例の臨床経過を振り返るとともに、STEP法の現況を把握し、今後の方向性を考察した。STEPは短腸症候群という病態であれば様々な年齢層の患者に適応があり、自験例では術中術後に大きな合併症を認めた症例なく、全例で経腸栄養の比率の増加と肝機能、特にGOT、GPTの正常化が認められ、2例では腸内細菌叢が正常化した。また、術後に消化管出血を認めた1例は輸血にて改善し、腸管の再拡張を認めた2例ではrepeat STEPや静脈栄養の再開が必要であった。短腸症候群の患児にとって、STEPは腸内細菌増殖の抑制、肝機能障害の是正、肝不全の予防という観点において非常に大きな治療の礎であることに間違いはなく、腸管再拡張に対してはrepeat STEPが適応と考えられた。

小児卵巣腫瘍茎捻転に特異的な臨床症状はない。時に骨盤内膿瘍との鑑別が困難な場合がある。超音波検査所見に基づき正しい診断が得られ、準緊急手術を行った6歳女児例を報告する。右下腹部痛を主訴に前医を受診し、CT上、右骨盤腔に膿瘍と糞石像を指摘された。Interval appendectomy目的で同日深夜当院へ転院した。腹部症状は軽度で、翌朝、超音波検査で再評価を行ったところ、CTで膿瘍腔と思われた壁内に卵胞様構造(multiple peripheral cyst)がみられた。近傍にtarget状に描出される別の小円形腫瘤(extraovarian round mass)をみとめ、whirlpool signが陽性であった。以上より右卵巣奇形腫茎捻転と診断し、腹腔鏡下に所見を確認して手術を行った。右卵巣腫瘍茎捻転と腫瘤形成性虫垂炎の画像所見は擬似することがあり、CTよりもむしろ超音波所見から多くの情報が得られる場合があることが示唆された。(著者抄録)

小児がん検診と関連して、自験例88例(2003〜2008年)を対象として小児がんの初期診断過程における超音波検査の実態を分析した。超音波検査は79例(88.8%)で施行されており、偽陰性が2例、診断的有用性が限定的とされた症例が10例みられたが、約半数の症例では腫瘍診断の直接のモダリティとなっており、その他の症例でも画像評価の一環として有用性が示された。10例では腫瘍は出生前診断され、子宮内胎児死亡の回避に有用であった症例も経験された。これらの結果ならびに小児がんの特異な生物学的特性から、超音波検査は成人がんと同じかたちでの小児がん対策型検診への導入は困難と思われるが、多臓器、多病態のスクリーニングとしての検診システムへの展開が期待できるものと考えられた。(著者抄録)

BACKGROUND: The etiology of Kawasaki Disease (KD) is enigmatic, although an infectious cause is suspected. Polymorphisms in CC chemokine receptor 5 (CCR5) and/or its potent ligand CCL3L1 influence KD susceptibility in US, European and Korean populations. However, the influence of these variations on KD susceptibility, coronary artery lesions (CAL) and response to intravenous immunoglobulin (IVIG) in Japanese children, who have the highest incidence of KD, is unknown. METHODOLOGY/PRINCIPAL FINDINGS: We used unconditional logistic regression analyses to determine the associations of the copy number of the CCL3L1 gene-containing duplication and CCR2-CCR5 haplotypes in 133 Japanese KD cases [33 with CAL and 25 with resistance to IVIG] and 312 Japanese controls without a history of KD. We observed that the deviation from the population average of four CCL3L1 copies (i.e., four copies) was associated with an increased risk of KD and IVIG resistance (adjusted odds ratio (OR)=2.25, p=0.004 and OR=6.26, p=0.089, respectively). Heterozygosity for the CCR5 HHF*2 haplotype was associated with a reduced risk of both IVIG resistance (OR=0.21, p=0.026) and CAL development (OR=0.44, p=0.071). CONCLUSIONS/SIGNIFICANCE: The CCL3L1-CCR5 axis may play an important role in KD pathogenesis. In addition to clinical and laboratory parameters, genetic markers may also predict risk of CAL and resistance to IVIG.

This report describes a case of fetal mediastinal teratoma complicated by hydrops fetalis managed successfully by aspiration of the tumor cyst fluid. Fetal mediastinal teratomas are rare tumors that cause hydrops fetalis or fetal demise in the prenatal period and respiratory distress in the neonatal period. The patient presented with a large cystic mass in the thoracic cavity complicated by hydrops fetalis. The hydrops resolved after fetal aspiration of the tumor cyst fluid. The infant was born without respiratory distress, and tumor resection was performed at the age of 30 days. The postoperative course was uneventful, and the patient was in good health 6 months postoperatively.

2002年3月〜2009年12月の新生児408例中、腫瘍性疾患34例(女児20例、男児14例)を対象に、診療録、画像診断などについて後方視的に検討した。出生体重は腫瘍を含んだ体重で700〜4338g、在胎週数は25週〜38週で、平均値の差のt検定など統計学的解析を施行した。対象症例中、最も多かったのは仙尾部奇形腫で12例(35.3%)を占め、次いで卵巣嚢腫10例(29.4%)、リンパ管腫4例(11.8%)が続いた。奇形腫は縦隔原発が2例、縦隔・後腹膜、後腹膜、頸部原発が各1例で、仙尾部奇形腫と合わせて全体の50%を占めた。その他、副腎腫瘍や巨大な肝血管腫各1例が含まれた。仙尾部奇形腫では全例が帝王切開で出生し、12例中1例は母体の中毒症状が発現したため緊急帝王切開となった。頸部リンパ管腫、奇形腫も帝王切開が選択され、奇形腫の1例はEXITで気道が確保されていた。後腹膜および縦隔原発の奇形腫は帝王切開と経腟分娩が半数ずつ選択され、卵巣嚢腫は全例が経腟的に出生していた。その他の腋窩のリンパ管腫も腫瘍の大きな1例で帝王切開が選択されたが他は経腟的に分娩された。肝の巨大な血管腫は胎児の著明な腹部膨満のため帝王切開が選択された。対象症例中、出生前に外科的な介入がなされた症例は2例(5.9%)のみで1例は仙尾部奇形腫で胎児水腫を併発したため腫瘍血管床減少のため腫瘍内部のラジオ波焼灼が行われた。もう1例は頸部奇形腫でEXITにより気道が確保された。

著者らが経験した先天性食道閉鎖症23例を対象に、合併奇形と治療戦略について検討した。その結果、生存例は19例(82.6%)、死亡例は4例(17.4%)であった。今回の検討では予後に直接的に影響したものは重症心大血管奇形と染色体異常であった。重症心大血管奇形を合併した症例では小児循環器医、小児循環器外科医との密接な連携により肺血流量の至適化と循環動態の安定をはかり、心予備能に見合った侵襲の小児外科手術を選択することが重要であると考えられた。
