Interoception, the sense of the internal state of the body, plays an important role in anxiety disorders and other psychiatric disorders, as well as in neurological disorders that cause pain. This effect is likely to occur in situations where anticipatory processing is possible. However, the mechanisms behind these phenomena remain unresolved. We first conducted a study to develop an accurate autonomic index and showed that facial skin blood flow could be an appropriate marker to reflect the fine state of mind. The results of this study may be applied to the treatment of blushing disorder or other anxiety-related disorders. The results of other experiment for the patients with chronic cephalalgia showed that the interoceptive accuracy tended to be lower in the patients, suggesting that it relates to the intensity of pain felt, not the frequency of pain.

In this study, we analyzed how autonomic function is altered by insular gyrus removal. Furthermore, by directly stimulating the insular cortex during awake surgery, measuring ECG, heart rate and blood pressure changes, and performing a heartbeat detection task, the aim of this study was to elucidate the neural basis of autonomic functional activity in the insular cortex and establish a next-generation awake brain surgery method to identify autonomic function, preserve function and restore function over time. The performance of direct stimulation and heartbeat detection tasks, as well as their relationship to the surgical site, was investigated in a statistical analysis. Analysis is currently underway in 13 patients who were able to be enrolled, and it has been found that cardiac output tends to decrease as a result of the decrease in systolic blood pressure and increase in peripheral vascular resistance caused by insular stimulation.

This study attempted to elucidate how people control their minds by using the findings and research methods of cognitive research. In particular, we focused on the suppression of episodic memory as a representative phenomenon of the control function. We examined the mechanism of memory suppression, individual differences, envisioning future events, and memory monitoring. We used the Think/No-Think (TNT) task and a retrieval-induced forgetting to examine the function of inhibition of memory information. We found that inhibition occurred in a sequential memory task and the effect of retrieval of generalized memories on retrieval of specific memories. Various approaches to elucidating cognitive control were found to be significant.

我が国では気分障害の患者数が非常に多く，その原因となるメカニズム解明が求められている．実際，さまざまな療法では症状の改善が見られない「ノンリスポンダー」も一定数含まれる．その背景には，自律神経の応答性に個人差がある点，および自らの心臓の動きなどの自律神経活動の変化をどれだけ正確に感じ取ることができるかを示す指標である「内受容感覚」にも個人差がある点が関与している．また生活環境内には，気圧の変化など，自律神経活動を大きく作用する要因がある．そこで本研究では，気圧変化が気分変化とどのような関係にあるかについて，「心ー脳ー身体ー環境」の4者関係の複雑なメカニズム解明を通して明らかにする予定である． 本研究は，精神疾患における気分のゆらぎと身体化の原因について，生活環境要因と心理的要因の効果を統合的に分析することを目的としている．気分の揺らぎはさまざまな原因によって起こる上に，身体化の背景には非常に複雑な要因が関わるため，身体生理機能・中枢神経機能に加え，主観的指標・客観的指標の両者を用いた心理的機能に着目する必要がある．今年度は，気圧の変化およびそれに対する内受容感覚に着目した実験を継続的に実施し，健常者および身体症状症患者の両群ともに多数のデータ取得ができた．さまざまな部位に痛みが生じている患者に対して，複数の質問紙による主観報告を実施し，意思決定や感情処理のバイアスかあるか，痛み閾値に特殊性が見出されるか，痛みの感じ方に違いはあるか，などの側面について徹底的な解析を行い，さらに気圧変化時の自律神経動態について詳しく調べた．また自律神経に変化が生じていない状態および生じている状態における内受容感覚を詳細に調べ，主観・客観の両側面から多角的な実験データが取得できた．その結果，内受容感覚を複数の次元に分けた分析結果で興味深いデータが得られ，患者の痛みなどの感覚がなぜ生まれるのかの機序の一端が明らかにされた．さらに，痛みの生起に，身体感覚増幅傾向や身体の内受容感覚が関与していること，それらが自律神経応答特性の影響を受けていることなどが示され，心理的痛みの背景にあるメカニズムが部分的に明らかになりつつある． 今年度は順調にリクルートが進み，順調なデータの取得ができている．臨床研究はさまざまな要因によって実験が計画通りに進まないことが多いが，本研究はその点ではおおむね順調に実験が進められており，データ解析も計画通りに進行している．このペースで次年度の研究も進める予定である． 上述のとおり，本研究の遂行はおおむね順調であり，今後もデータ解析，および他の実験を並行して進める予定である．データが安定している部分については，積極的に学会での発表などを行い，専門家の意見を広く取り入れ，データの解釈に活かしていく予定である．

我が国で頭痛やめまいの症状を持つ患者数は3,000万人を超えると言われており，それに伴う気分の揺らぎに伴うパフォーマンスの低下は，社会全体の活動低下を招いている．これらの患者のなかには，心的ストレスが原因となって身体の不調を訴える症例が多く含まれているが，なぜそのような身体化の症状が生じるかは十分に解明されていない．本研究では，健常者・身体症状症・片頭痛・メニエール病の患者を対象として，自律神経活動，身体状態の変化を感知する内受容感覚，予測的情報処理に基づく信念形成の特性に着目し，気圧変化に対する身体・認知特性を明らかにする．そして，症状改善に向けたプロトコルの開発に取り組む．

Chronic orofacial pain involves central sensitization, in which the brain amplifies pain signals. Pain is processed as intravital information, i.e. an interoception. In patients with chronic pain, stronger central sensitization decreased the accuracy of interoception and the inhibitory/excitatory ratio of neurometabolites in the left insular cortex. Brain activation during the perception of interoception and pain imagery tasks showed that chronic pain patients exhibited different patterns of activation than normal subjects in the regions associated with interoception and chronicity of pain. It was suggested that excitatory/inhibitory neurometabolites were involved in the background.

この研究において、自律神経機能の同定と機能温存、経時的な機能回復を目的とした次世代型覚醒下手術法の確立を目標とする。また、自律神経系ネットワークの早期回復を目指し、脳腫瘍患者の術後の生命予後の改善のみならず、機能予後改善を図ることを主眼に置いた治療を行う。 眼窩前頭皮質が自律神経と密接に関わることをヒトで検討できる脳神経外科学と、ヒトの感情や自律神経機能を多角的に捉えることができる認知神経科学が、分野融合型の共同研究を行うことは大変意義深いと考える。 眼窩前頭皮質の損傷によって、他者の考えや感情を推し量ることが苦手となり、社会的行動に障害をきたすことが多いとされる。この背景には、自律神経機能低下が関わることが示唆される。また眼窩前頭皮質に及ぶ脳腫瘍切除後に、食欲の低下、摂食障害となる症例が多く、眼窩前頭皮質と自律神経ネットワークに関連性がある可能性に着目した。しかしながら、眼窩前頭皮質が自律神経機能の制御と監視に関連するという断片的なデータは集まってきているが、詳細な関連性についての実証研究はされておらず、眼窩前頭皮質における自律神経機能や役割については未だ分かっていないことが多い。また、デフォルトモードネットワークの一部である眼窩前頭皮質は内受容感覚に関わるとされているが、ヒトの眼窩前頭皮質の自律神経機能活動の中心が未だ交感神経活動なのか副交感神経活動なのか分かっていない。覚醒下手術中に眼窩前頭皮質を直接刺激し、心電図、心拍数、血圧変化の測定、心拍検出課題を行うことで、眼窩前頭皮質における自律神経機能活動の神経基盤を解明し、自律神経機能の同定と機能温存、経時的な機能回復を目的とした次世代型覚醒下脳手術法の確立をこの研究の目的とする。 覚醒下手術において、実際に眼窩前頭皮質を直接刺激する、及び、術前術後の心電図、心拍数、血圧の変化を測定すること、さらに心拍検出課題データの解析を行った。今年度は、覚醒下機能マッピングによる眼窩前頭皮質への刺激の条件設定、刺激方法の確立を目指す方向で進んでいる。眼窩前頭皮質を刺激する詳細な部位、刺激強度、課題の種類の選定や課題施行回数の詳細な条件を決定できた。また内受容感覚の鋭敏さの測定方法としての心拍検出課題についても条件設定を行ってきた。決定した条件を基に、眼窩前頭皮質に存在する脳腫瘍の患者に、覚醒下手術中および術前術後の心電図、心拍数、血圧の変化を測定しデータを蓄積している。 症例登録は順調にいっている。 症例登録を行い、解析を進めていく。

Recent studies have investigated the neural mechanisms of emotion and social functions. However, causal factors of anxiety and depression symptoms remain unclear. In this series of studies, we have focused on the mind-brain-body interactions from integrative multiple perspectives and carried out three experimental investigations including an EEG study, an fMRI study, and an autonomic clinical neurological study. Results indicated that specific patterns of automatic thinking including past- of future-oriented thinking were found in depression and anxiety, which relates interoceptive accuracy and autonomic nervous functions. For the neural mechanisms of these specificities, we found the early component of EEG and the activity in the frontopolar area could be the keys to understand anxiety and depression symptoms. Overall findings suggest that we need to address the mind-brain-body interactions for uncovering the behavioral and neural mechanism of psychiatric symptoms.

Somatic symptoms disorder, in which patients complain of pain and other physical symptoms even though there are no obvious symptoms of physical disease, actually occurs in a considerable number of cases, and certain types of psychotherapy are found to be effective. However, the mechanisms underlying their symptoms are not fully understood. In the present study, we focused on three factors highly involved in the occurrence of symptoms: predictive autonomic nervous functions at the body level, interoceptive accuracy, and distortion of thought, and examined them in detail. The results showed that similar changes in autonomic nervous activities were observed not only in the situation of actual atmospheric pressure fluctuations, but also in the situation of merely predicting them, and that the degrees of those changes were partially associated with interoception, somatic symptom characteristics and the degree of distortion of thought.

研究プロジェクト全体の目的は，未来性に関わる課題遂行中の脳活動および自律神経活動の測定を通して，「心・脳・身体」という三者関係から，不安障害や抑うつ障害と関係の深い未来思考性について検討することである．これまでの研究では「心の未来性」のメカニズムにおける「心・脳・身体」のダイナミクスの理解を目指し，文章の刺激を用いてポジティブあるいはネガティブ方向への自動思考性を引き出すような状況を作り，未来思考性と過去思考性における処理をターゲットとして，脳波計を用いた検討を行った．そして，心拍誘導電位を指標とした実験結果から，脳活動が身体由来であることを突き止めることができた．本年度はその成果を受け，さらに潜在連想テスト(IAT)を用いた検討を行った．そして，未来思考の特異性を呈する不安障害傾向と，過去思考の特異性を呈するうつ病傾向の違いを，時間方向に依存した思考と概念形成という新しい枠組みで捉えることを試みた．その結果，一部の指標において，仮説を支持する結果が得られ，初年度の成果と整合する結論を導き出すことができた．また，うつや不安とは逆の軸として位置づけられる楽観主義傾向についても行動実験を実施し，楽観傾向の背後にある行動的メカニズムを明らかにすることができた．これらの総合的な成果は，心の時間的な側面に関する基礎的，かつ臨床的な意義を持っており，時間と思考方向性に関する実験的パラダイムの確立に結びつけることができた．これは，心理学的にも神経科学的にも重要な知見であり，さまざまな方向性に発展，展開できる可能性を持つ重要な成果であると判断する．

Unlike physical time, "mental time" as perceived by the brain can expand, contract, and segment at will depending on internal conditions such as attention, motivation, and context. This mental time is often inappropriately transformed and sometimes deeply implicated in disease. This project will bring together a comprehensive team of neuroscientists, cognitive neuroscientists, and clinical neuropsychologists to elucidate how psychological time disintegrates and becomes disease. We will reveal the contours of normal and pathological temporal functions of the brain through human clinical trials and animal studies.

「退屈」という状態は「自分を満足させる活動に、従事したくともできない嫌悪的状態」と定義される。退屈を避けるためにヒトは嫌悪刺激を進んで受ける場合があることが知られているが、そのメカニズムは明らかではない。そこで本研究ではまず、神経活動の記録や操作が容易なマウスを用いて退屈度を定量する行動試験系の構築を試みた。その結果、ヒトと同じように、マウスも刺激の少ない環境で嫌悪刺激を自発的に受ける「退屈様行動」を示すことが明らかになった。この行動試験系を用いることで、退屈様行動に相関した神経活動および、報酬系を担う脳領域である腹側線条体のドパミン動態を記録した。まず、マウスの退屈を反映した行動がみられるかを検証した。ヒトが退屈な状態で自らに電気ショックなどの嫌悪刺激を与えるという知見を参考にして、嫌悪刺激を自発的に受けられるような実験箱にマウスを入れる行動試験系を構築した。具体的には、箱内の穴にノーズポークをすると、嫌悪刺激として、穴から空気を勢いよく当てるエアパフを起こす装置を作り、遊具のあり/なしでポーク数の比較を行った。すると遊具なし条件において2.1倍多くポークし、嫌悪刺激を受けに行くことが明らかになった。次に、マウスが嫌悪刺激のエアパフを自発的に受けに行くメカニズムに迫ったウイルスベクターを用いて、ドパミンセンサーの一つであるGRABDA2mを腹側線条体に発現させ、光ファイバーを通じて行動試験中のドパミン濃度を記録した。するとポークの直前にドパミン濃度が一過的に上昇し、ポーク後は速やかに減少した。この変化はポークをしてもエアパフの出ない条件では見られず、また受動的にエアパフを与えた時にはエアパフ直後のドパミン濃度の減少のみがみられた。これらの結果から、エアパフ自体は嫌悪的に感じているものの、退屈様行動においてはエアパフへの期待が報酬となっている可能性がある。

脳で認識される時間情報がどのように神経細胞で表現され、またそれが環境や状況に応じてどのように変化したり崩壊したりするのかを解明するため、げっ歯類動物での行動神経生理学的アプローチにより研究を進めている。とくに、数秒から数十秒の時間知覚であるインターバル計時に着目し、神経細胞による時間経過情報の表象・変容に注目した研究を進めてきた。 ヒトの研究では、1) 精神疾患(うつおよび不安)における思考の時間的方向性の特徴を調べる実験を実施した。自己と他者を行為者とする条件を設定した上で思考の未来性と時間感覚の関係性について調べ、自己に関する処理の特殊性をあぶり出すことができた。また、2) うつ病患者における未来思考が努力性であることを確認し、認知行動療法を受けた後に、それが改善される際の神経基盤をMRIを用いて調べ、症状改善の度合いが前頭極の認知処理と深く関連することを明らかにした。さらに、3) 楔前部を含む頭頂葉内側部の記憶機能について、それが認知症進行のバイオマーカーであるアミロイドβおよびタウ蛋白の蓄積量とどのような関連があるかを調べる研究を進めた。その第一段階として、安静時fMRIの時間変動解析の結果、楔前部の活動が記憶の反応カテゴリーと強い関連があることなどが明らかになった。またアルツハイマー型認知症および発達障害を対象とし、臨床神経心理学の研究手法を用いてこころの時間の解明を目指すなかで、今年度は（A）自閉症の時間知覚の特徴、（B）アルツハイマー型認知症における時間判断課題の傾向、および（C）時間産出・時間再現課題と脳血流の指標であるSPECT値の関連の3点について検討を行った。以下３点の研究成果が得られており、研究は滞りなく進捗している。

In cognitive psychology, it is considered that both unconscious/ automatic processing and intentional/ conscious processing play an important roles in the human mind. Especially, there have been lots of researches emphasizing on automatic processing. On the other hand, people usually feel to control their own mind, but sometimes there are situations which they cannot control one’s own memory and thought, for example, intrusive thought enters into our consciousness though we don’t want to think about it. In this project, we sought to elucidate the mechanism of cognitive control, mainly based on memory suppression, intentional forgetting. Eventually, we found that people can suppress their unwanted memory and that the prefrontal cortex is important in the control process.

○We assessed emotion processing in a series of patients who had undergone brain tumor resection in the insular lobe. We found that direct electrical stimulation over the anterior insular cortex during awake brain mapping enhanced anger recognition in these patients. ○The sensitivity of anger recognition significantly deteriorated, while that of sadness improved, when emotional sensitivity was compared before and after surgery. ○We showed that anger recognition is mainly associated with the left insula in a lesion symptom mapping study.
