The mechanism of food-drug interactions includes the inhibition of metabolic enzymes or drug transporters, which have genetic variations. The aim of this study is to clarify whether the genetic variations of metabolic enzymes and transporters lead to the altered inhibitory kinetics of respective inhibitors contained in a variety of fruit juices. We focused upon two metabolic enzymes (CYP2C9 and CYP2C19) and two transporters (OATP1A2 and OATP2B1). Detailed analysis revealed that the inhibitory kinetics of various inhibitors highly differ among genetic variants. Therefore, the genetic variants of metabolic enzymes and drug transporters are considered to be one of the important factors to confer inter-individual variation in the extent of food-drug interactions. We also succeeded in identifying a novel OATP inhibitor, narirutin, from grape fruit juice.

There seems to be considerable inter-individual variations in the extent of drug interactions, especially via the inhibition of oxidative metabolic enzymes, P450s. This study aimed to investigate quantitatively the influence of genetic variation of P450s, such as CYP3A4 and CYP2D6, on the extent of drug interaction in the in vitro studies using genetic variants. As a result of enzymatic study with probe substrates and mechanism-based inhibitors (MBI), we concluded that the inhibitory potencies of MBI differ among genetic variants. These results suggest that the genetic variation of metabolic enzymes affect the extent of drug interactions caused by their inhibitors, as well as the pharmacokinetics of their substrate drugs.

母体に投与された薬物などの生体異物は胎盤を介して胎児へと移行するため、薬物の胎児毒性を評価する上では、その胎盤透過性を評価することが必要不可欠である。本研究においては、ヒト満期胎盤を用いて、母体側から、ならびに胎児側から、さまざまなプロトコルで薬物の胎盤灌流を行い、灌流流出液中の薬物濃度を経時的にモニターすることにより、薬物の経胎盤透過キネティクスを定量的に評価した。これにより、薬物の胎児移行性を動的にとらえることができようになった。また、ジクロフェナクと単純拡散マーカーであるアンチピリンの胎盤透過性に対する乳酸(MCT阻害剤)の影響を検討したところ、ジクロフェナクの胎児移行性が低下することが示され、胎盤灌流実験とモデル解析の組み合わせにより、薬物の胎児移行に関わる輸送担体の寄与を定量的に評価できる可能性を示すことができた。胎盤中での薬物の代謝については、胎盤ホモジネートを用いたpreliminaryな代謝実験を行ったが、アンチピリンの有意な代謝は検出されなかった。また、ヒト満期胎盤よりトロホブラスト細胞を単離精製し、エストロン硫酸をはじめ、各種物質の取り込み特性をキネティカルに評価することができた。しかしながら、単離した細胞を単層に培養し物質の経細胞輸送を評価する試みは成功しなかった。単離生成したトロホブラスト細胞におけるOATPsの発現の経時変化をRT-PCR法により確認したところ、経日的に発現量は大きく変動したが、その推移は分子種間、ロット間で大きく異なり、一定の傾向を見いだすことはできなかった。

The influences of genetic variation of CYP3A4 on the inhibitory kinetics of its inhibitors varied among the inhibitors and affected by the probe substrate used. The inhibitor manners (patterns) of metabolic inhibitors contained in fruit juice are different among CYP isoforms, i.e. CYP3A4, 2C9 and 2C19. The research should be focused upon the mode of inhibition that is clinically most relevant. Using newly developed cell lines stably expressing a transporter OATP1A2 or OATP2B1, some unidentified inhibitory ingredient(s) for these transporters are considered to be contained in some fractions of grapefruit juice.

The aim of this study was to develop the computer-aided system that assists dosage regiment optimization, prescription checking, proper use of drugs and distribution of drug information based on the pharmacokinetics and pharmacodynamics. We developed algorithms for personalized pharmacotherapy with regard to 1) dosage adjustment of quinolone antibiotics and angiotensin converting enzyme inhibitors in patients with renal dysfunction, 2) quantitative estimation of time-dependent effects of St.John's wort on the drug metabolism and excretion, 3) receptor occupancy-estimation of antipsychotics-induced adverse reactions, 4) dose regime recommendations to avoid drug interaction between new-quinolones and metal cations, 5) estimation of dosing interval dependency of the interaction between itraconazole and triazolam, 6) assessment of the effects of α1-adorenoceptor antagonists in benign prostatic hyperplasia based on the receptor occupancy theory, 7) avoiding piloerection associated with antidepressants, and 8) optimization of the dosage regimen of TS-1 (a combination preparation of tegafur, gimeracil and oteracil potassium). We also developed prototype systems for some of the above algorithms on a personal computer. For example, with regard to the above 1), we developed a computer-aided system that helps to optimize the dosage and dosing time in patients with arbitrary grade of renal function by adjusting the patient's plasma concentration profile to that of subjects with normal renal function. Moreover, we inquired, by pharmacists' questionnaire, the needs for the computer-aided systems for dosage optimization after patients' poor compliance or alteration of the dosage form, and developed the prototypes. We also inquired into the standardization of drug information with regard to the dosage formulation to develop a computer-aided prescription checking system. (260 words)

In the present study, a novel approach to predict fetal toxicity induced by the use of non-steroidal anti-inflammatory drugs (NSAIDs) in full-term pregnant women was developed using the transplacental pharmacokinetic (TP-PK) parameters obtained from in situ human placental perfusion studies, and the pharmacodynamic (PD) parameters obtained from fetal plasma concentration-response relationship in rats. Using the novel approach, the fetal toxicity of NSAIDs such as ibuprofen was quantitatively predicted and the use of NSAIDs is suggested to be dangerous to full-term pregnant women.

本研究では、投薬に関するアクシデント・インシデント事例を大規模に収集し、医学・薬学的側面のみならず、人間科学的・社会科学的側面からも精細に分析し、それらを分類・整理した「薬物治療に関するアクシデント・インシデントライブラリー」を構築すること、加えて、ライブラリーを活用して、事例間の共通点やパターン、法則性を解析することを目的とした。今年度は、昨年度確立した分析法を用いて、服薬に関するアクシデント・インシデント事例の収集、分析、データベースへの格納を行った。具体的には、分析対象とした210事例について、事象の整理(関与者の認知・判断・行動の時系列の書き出し)、問題点の抽出(先の事象から問題点を抽出)、背後要因(問題点惹起の要因の抽出、分類)の探索を行い、データベースに格納した。問題点や背後要因は詳細なカテゴリ分類項目(昨年度開発)を選択することで分析した。構築したライブラリーを活用して、事例解析を行った結果、対象とした全事例で複数の背後要因が見られ、ミスの要因となった医薬品特性、患者特性、医療従事者特性の詳細解析を行うことで事例間の共通点やパターン、法則性が明らかになることが示唆された。今後、構築したライブラリーに継続して新しい事例を格納していくことでアクシデント・インシデント事例の未然回避につながることが期待される。さらに本ライブラリーは医療従事者の研修ツールとしての効果も期待されるだろう。

Syndrome maline with clinical futures such as fever, myoclonus, hidropoiesis and mental status changes is recognized as a severe adverse reaction. Recently, the syndrome maline induced by antidepressant has been also reported in human beings. The purpose of the present study is to prepare the syndrome maline model rats, to induce fever which is one of pathognomonic symptom of the syndrome maline after administration of several serotonin (5-HT) reuptake inhibitors to the model rats, to elucidate the relationship between pharmacokinetics of antidepressant and 5-HT levels in brain, or elevated fever. Final goal is In addition. In 1997, Firstly, we developed a high performance liquid chromatographic method for determination of antidepressant, clomipramine, in biological samples. Clomipramine concentrations in plasma and brain after intravenous administration of the drug to intact rats were measured by the analytical method, and the pharmacokinetics of clomipramine was analyzed. Then, clomipramine was administered intraperitoneally to rats after veratrine, a neurotransmitter releasing agent, was injected to the PO/AH.By the administration method, we could reproduced the elevated fever as a marker of the syndrome maline. In 1998, we measured 5-HT level in fat brain after coadministration of veratrine and clomipramine. The 5-HT level in brain with veratrine we higher than that without.

投薬ミスに代表される医療ミスは大きな社会的問題となっている。その要因として医薬品の製剤や包装の特性や、その変更が関係した事例が多数見出されている。本研究では、こうした医薬品の製剤・包装の特性やその変更に伴う投薬ミスの要因を詳細に解析するとともに、製剤や包装、その変更にかかる情報を一元的に管理し有効活用するためのシステムの構築を目指した。 本年度においては、昨年度までにリストアップした製剤・包装に関する特性項目(錠剤、散剤、カプセル剤、液剤、龍用剤、外用塗布剤、貼付剤に関する特性階層項目)に関するデータベースのブラッシュアップを行った。また、昨年度までに構築した包装・製剤変更情報ハンドリングのためのシステムのプロトタイプを、WEBブラウザを介して編集するためのシステムを、インターネットの商用サーバ上に構築した。続いて実際に医療現場に発出された包装・製剤変更に関する情報をシステムに入力し、構築したシステムの実用性に関して評価し、システムの修正作業を行った。最終的な実用性の評価は、管理者、データを入力・編集する製造企業、データを利用する医療従事者、それぞれの立場に関してアカウントを設定して実施した。その結果、構築したシステムは、それぞれの立場から、インターネットを介して、実際に行われた包装・製剤変更の情報を有効に規格化・整理して共有できることが確認された。

本研究では健康食品の情報不足を解決するために、健康食品の有害事象や安全性情報を積極的に掘り起こし創製していくことを目的として、薬剤師間及び医師間健康食品情報収集・交換システムを構築した。昨年度に引き続き、医療従事者のニーズを考慮した健康食品情報(健康食品素材に関するモノグラフと医薬品と健康食品の相互作用に関する事例)を作成し、構築したシステムを介して登録者に向けて情報提供を行った。医師・薬剤師の知識・意識を高めるために、健康食品に関する定期的な情報提供、事例形式の情報、規格化された情報が有用であることを示した。システム内では、情報提供と並行して、医師・薬剤師から健康食品による健康被害情報を収集し、調査・評価したのち登録者間で情報共有するためのコーナーを設置した。簡易化した投稿機能も設けることで、医師・薬剤師からの類似事例の収集を促進させることにも成功した。さらに、本システムを介して収集された事例に基づき、グルコサミンサプリメント中のカリウム含有量の分析を行った。その結果、一部のグルコサミン硫酸塩含有サプリメントには高用量のカリウムが含まれていることが明らかになり、カリウム摂取制限が必要な患者は当該サプリメントの摂取に注意が必要であると考えられた。以上から、本システムを活用することで医療現場から健康食品情報を収集し、早期に共有・類似事例を蓄積することが可能となり、それらの情報に基づき、健康食品の科学的エビデンス作りにも寄与できる可能性を示した。

平成17年度は,以下の4項目について研究した. (1)薬名間の類似性指標の設計(竹田,坂内) 前年度に引き続き,薬名間の類似度を定量化する指標を設計した.各パラメータを決定するため,研究室現有のタキストスコープ装置を用いて視覚および聴覚に関する知覚誤りの実験を行った.得られたデータに機械学習アルゴリズムを適用しパラメータの学習を行った. (2)類似性指標の検証(澤田) 日本薬剤師会などから報告されている「取り違え事例」(日本薬剤師会雑誌53(Sup 4):60-63,2001)や,澤田らがインターネット上に運営している薬剤師間コミュニティーサイト(薬学雑誌122:185-192,2002)に寄せられた事例の医薬品等をもとに,(1)の類似性指標の妥当性を検証した. (3)類似薬名検索方式の開発(篠原,竹田) (1)(2)の成果をもとに,与えられた医薬品名に対して高い類似度を示す医薬品を,医薬品データベース中から高速に検索するためのアルゴリズムを開発した.これを,WS上に実装し,その性能を評価した. (4)処方関連度辞書の構築(大谷,畑埜) 医薬品の処方学的関連の度合を数量化した医薬品処方関連度辞書の構築を目指し,大量の処方箋データからのデータマイニング方式について検討した.

後発医薬品は患者の負担を軽減し、国の医療費削減に貢献する有効な対策として、その使用が推進されつつある。本研究においては、医薬品情報を収集し、それをもとにジェネリック医薬品の評価・格付けを行うための方法論を確立し実証することを目的とした。昨年度は、評価系を構築するとともに、モデル薬剤としてラニチジンを選択し、13社の製品ついて評価を行った。そこで本年度はまず、薬剤師を対象にアンケート調査を実施した。その結果、365名の薬剤師より回答を得て、ジェネリック医薬品に関して医療現場が望んでいる情報を明らかにすることができた。続いて、このアンケート調査の結果と、前年度に行った評価の結果とを考慮に入れて評価系の見直しを行った。そして、評価項目を47項目とし、(1)生物学的同等性に関する項目(9項目)、(2)作用に関する項目(4項目)、(3>製剤等に関する項目(9項目)、(4)添付文書の整備状況(7項目)、(5)その他の文書の整備状況(13項目)、(6)情報サービスの質(5項目)、という6つのカテゴリーに分類した。 最後に、モデル薬剤としてプラバスタチン製剤を選択し、先発品と27種の後発医薬品、計28種について、販売企業に文書により情報の提供を依頼し、上記の項目の評価を行った。構築した基準による評価の結果、各製剤の総合的評価や個々の特性を俯瞰できた。また、各カテゴリー間の独立性は高く、本評価法は、個々のGEの特性、すなわち長所や短所を一目で把握する上で有用であった。 今回構築した新規評価法は、後発医薬品間の総合的評価とともに、個々の特性を把握する上で、極めて有用なツールであることが明らかとなった。本評価系を活用することで、薬局をはじめとする医療機関が、最も優れた後発医薬品を採用し、自信を持って患者に後発医薬品を提供することができるようになるだろう。(750)

The hemagglutation virus of Japan (HVJ)-liposomes were found to be a useful tool for the efficient introduction of macromolecules, including oligodeoxynucleotide, into cultured brain capillary endothelial cells in vitro. 2. Using HVJ-liposomes encapsulating a β-galactosidase gene expression vector, the gene was demonstrated to be efficiently transfered into cultured brain capillary endothelial cells in vitro. 3. The efficient transfer of gene into brain capillary in vivo was achieved by the administration of the HVJ-liposomes encapsulating a β-galactosidase gene expression vector into carotid artery. The expression of β-galactosidase at brain capillaries was found throughout the parenchyma. 4. The above results (2 and 3) indicate that the HVJ-liposome is a useful vector to transfer foreign gene into the brain capillary endothelium in vitro and in vivo. 5. Additional study demonstrated that the cultured mouse brain endothelial cells transfected with mouse glial-derived neurotrophic factor (GDNF) cDNA was found to secrete mature GDNF predominantly to abluminal side (brain side).
