The procedure is the same as for a new application. Those scheduled for renewal will be sent a notification approximately one month before the application period begins. Please ensure that all required fields are completed before submitting your application.
If you have lost your Certified Pharmacist Certificate, you can request a reissuance with this application form. However, please note that the application fee will be the same as for a new or renewal application.
To apply for credits for a published paper, please complete this application form, prepare the necessary documents, and mail them to the Keio University University Faculty of Pharmacy Certified Pharmacist Training Program Committee.

For those who require a "Certificate of Attendance" for a training session, please complete this application form, enclose a self-addressed stamped envelope, and mail it to the Office of Lifelong Learning, Keio University University Faculty of Pharmacy. A Certificate of Attendance is not required for our faculty's credit stickers issued on or after September 1, 2019, on which your name and pharmacist registration number are pre-printed. How to Obtain the Keio University University Faculty of Pharmacy Certified Pharmacist Training Handbook
Obtaining by Mail
Please transfer a total of 1,200 JPY, which includes the price of the Certified Pharmacist Training Handbook (1,000 JPY) and a shipping and handling fee (200 JPY), to the postal transfer account listed below. We will mail the handbook to you upon confirmation of your payment.
Note: Even when purchasing multiple copies at the same time, the shipping and handling fee is charged per copy. Therefore, the price is 1,200 JPY per copy.

Those who have obtained "Keio University University Faculty of Pharmacy Certified Pharmacist" certification will receive one complimentary copy of the "Certified Pharmacist Training Handbook." It is enclosed with your pharmacist certificate, so please be careful not to purchase a separate copy.
