Applications will be submitted online through the Medical National Examination Online Procedure System (Me-LeX). Please register the information required to use Me-LeX in the form below.

Please mail the following five items to "Keio University Faculty of Pharmacy Office of Student Services, National Pharmacist Examination Coordinator."
Acceptance Period: Monday, November 2 – Friday, November 20 (Must arrive by this date)
Application form (prescribed form)*Please fill in the required information.
A copy of a photo ID (one of the following: driver's license, passport photo page, My Number card (within validity period, front side only, may be left in its dedicated case), or Residence Card (both sides))
*Please attach it to the designated space on "1. Application form (prescribed form)."
One photograph for the examination (6 cm high x 4 cm wide)
*Taken within six months prior to application.
*Please write your name on the back.
A 500-yen Postal Money Order (Certificate of Graduation issuance fee for the National Examination application)
*Please do not write anything on it.
Kaku-gata No. 2 envelope (for return mail, size that fits A4 paper without folding)
*Clearly write the applicant's name and address on the front, mark it as "Registered Mail" (Kan-i Kakitome) in red ink, and attach 530 yen worth of postage stamps. Folding is permitted.
■ Points to Note
The Certificate of Graduation for the National Examination application is different from a regular Certificate of Graduation, so it cannot be requested through any procedure other than the one described above.
If you wish to apply for "Examination Accommodations," please check the Ministry of Health, Labour and Welfare website below and apply directly to the Ministry by the specified deadline.

At the Shiba-Kyoritsu Campus, entry and use of facilities are not permitted for anyone other than students, faculty, and staff belonging to the Faculty of Pharmacy and the Graduate School of Pharmaceutical Sciences. As graduates cannot enter the campus buildings, please apply by mail.
