I: March 2025 Graduates -> Bulk Application via Keio University
In early November, the university will send information to the address of the guarantor registered during your enrollment.
Please check the contents and mail the required documents to "Keio University Faculty of Pharmacy Office of Student Services, National Pharmacist Examination Section" within the designated period. Applications will not be accepted after the designated period.
II: Graduates from March 2024 or earlier -> Individual Application
Please mail the following five items to "Keio University Faculty of Pharmacy Office of Student Services, National Pharmacist Examination Section," with "Request for National Pharmacist Examination Application Documents" written in red on the front of the envelope.
Application Period: Tuesday, November 4 – Friday, November 28 (Must arrive by this date)
Application Form (Prescribed Form) *Please fill in the required information.
Copy of photo ID (driver's license, passport, etc.)
*Please attach to the designated space on "1. Application Form (Prescribed Form)."
1 photo for the examination (6 cm height x 4 cm width)
*Taken within 6 months prior to application.
*Please write your name on the back.
500 yen Postal Money Order (Fee for issuing the Graduation Certificate for National Examination Application)
*Please do not write anything on it.
Kaku-gata No. 2 envelope (for return mail, size that fits A4 paper without folding)
*Clearly write the applicant's name and address on the front, write "Registered Mail" in red, and attach 530 yen worth of postage stamps. Folding is allowed.

The Graduation Certificate for National Examination Application is different from a regular graduation certificate, so it cannot be requested through any procedure other than the one mentioned above.
At the Shiba-Kyoritsu Campus, entry and use of facilities are restricted to students, faculty, and staff belonging to the Faculty of Pharmacy and the Graduate School of Pharmaceutical Sciences. Graduates are not permitted to enter the campus, so please apply by mail.
