Examination Period: Saturday, February 22 and Sunday, February 23, 2025
I: Graduates of March 2024 → Group Application through Keio University
In early November, the university will send information to the guarantor's address on file during your enrollment.
Please review the contents and mail the required documents within the specified period to the National Examination for Pharmacists desk at the Keio University Faculty of Pharmacy Office of Student Services. Applications will not be accepted after the specified period.
II: Graduates from March 2023 or earlier → Individual Application
Please mail the following five items to the National Examination for Pharmacists desk at the Keio University Faculty of Pharmacy Office of Student Services in an envelope with "Request for National Examination for Pharmacists Application Documents" written in red on the front.
Application Period: Friday, November 1–Friday, November 29 (Must arrive by this date)
1. Application Form (Designated Form).pdf Please fill in the required information on the form.
2. A copy of your photo identification (driver's license, passport, etc.). Please affix it to the designated space on "1. Application Form (Designated Form)."
3. One photograph for the examination (6 cm high x 4 cm wide). It must have been taken within six months prior to application. Please write your name on the back.
4. A 500-yen postal money order for the issuance fee for the Certificate of Graduation for the national examination application.
5. One Kaku-2 envelope (large enough to hold A4 paper without folding). Clearly write your name and address on the front, write "Kani-kakitome" (simplified registered mail) in red, and affix 530 yen worth of postage stamps. The envelope may be folded.
Notes
・The Certificate of Graduation for the national examination application is different from a standard certificate of graduation, so it cannot be requested through any other procedure.
・Those applying individually should follow the instructions on the Ministry of Health, Labour and Welfare website (especially section 5, "Application Procedures") to ensure the procedure is completed correctly.
・At the Shiba-Kyoritsu Campus, individuals other than students, faculty, and staff belonging to the Faculty of Pharmacy and the Graduate School of Pharmaceutical Sciences are not permitted to enter or use the facilities. Alumni are not permitted to enter the campus, so please apply by mail.
