We will begin accepting applications for Certified Pharmacist certification on August 1. (Applications must arrive by the deadline of September 15.)
For the Certified Pharmacist application procedure, please carefully review the "Keio University Faculty of Pharmacy Certified Pharmacist Training Handbook" and the information on this website .

The Certified Pharmacist Certification Standards were revised on November 1, 2022. Please review them thoroughly before applying.
The main changes are as follows. Please be sure to check for other changes as well.

Please ensure that the application form is filled out completely.
*Please note that there has been an increase in omissions of the number of credits.
For the Keio University Faculty of Pharmacy Certified Pharmacist Training Handbook, please read the Certified Pharmacist Training Program Regulations, Certification Standards, and sample entries carefully, and fill in all required information accurately, ensuring there are no omissions or errors, such as your name.
A copy of the payment receipt must be attached to the back of the application form. This receipt can be used as your proof of payment, so please send a copy to the university.
For new applications, please attach a photograph to your resume. Please write your name on the back of the photograph.
The required documents must be sent by simplified registered mail.
Please be sure to write your name in the Training Handbook and keep it from being damaged. Applications with unsigned handbooks or incomplete handbooks (missing pages) will not be accepted.
If you are using credits from the Japan Pharmacists Education Center (G01) Pharmacist Education and Certification System (PECS), please print out your credit record from PECS and enclose it with your application documents. Even in this case, please ensure that all fields in your Training Handbook, such as "Date of Attendance," "Training Subject," "Summary," and "Hours of Session," are filled out completely.

*Providers other than the Japan Pharmacists Education Center that issue credits electronically also have procedures for printing your credit records. Please contact each provider for details.

If your proof of lifelong learning credits is in a format other than stickers (such as a credit certificate), or if you have any questions about the procedure, please contact us before submitting your application.
