Examination Dates: Saturday, February 17, 2024, and Sunday, February 18, 2024
I: For those who graduated in March 2023 → Group application through Keio University
In early November, the university will mail information to the guarantor's address on file from your time as a student.
Please review the contents and mail the required documents to the National Examination for Pharmacists desk at the Keio University Faculty of Pharmacy Office of Student Services within the specified period. Applications will not be accepted after the deadline.
II: For those who graduated in or before March 2022 → Individual application
Please mail the following five items to the National Examination for Pharmacists desk at the Keio University Faculty of Pharmacy Office of Student Services. Write "Request for National Examination for Pharmacists Application Documents" in red on the front of the envelope.
Application Period: Wednesday, November 1 – Thursday, November 30 (must be received by this date)
2. A copy of your photo ID (driver's license, passport, etc.). Please affix it to the designated space on the Application Form (item 1).
3. One photograph for the examination (6 cm tall x 4 cm wide), taken within six months prior to application. Please write your university name and full name on the back.
4. A 500-yen fixed-amount postal money order for the issuance fee of the Certificate of Graduation for the national examination application.
5. One Kaku-2 envelope (large enough to hold A4 paper without folding). Clearly write your name and address on the front, write "Kani-kakitome (Simplified Registered Mail)" in red, and affix 490 yen worth of postage stamps (corrected 2023/11/10). The envelope may be folded.
《Important Notes》
・The Certificate of Graduation for the national examination application is different from a standard certificate of graduation and cannot be requested through any procedure other than the one described above.
・If you are applying individually, please follow the procedures on the Ministry of Health, Labour and Welfare website (especially section 5, "Juken Tetsuzuki" [Examination Procedures]) to ensure there are no errors.

・Access to and use of facilities at the Shiba-Kyoritsu Campus are restricted to students, faculty, and staff belonging to the Faculty of Pharmacy and the Graduate School of Pharmaceutical Sciences. Alumni are not permitted to enter the campus, so please apply by mail.
