◆ For graduates of March 2022 → (Application through the university)
Based on the information that will be sent from the university in early November, please prepare the necessary documents and mail them to the Office of Student Services during the specified period. (The information will be sent to the guarantor's address on record during your enrollment.) Please be aware that applications will not be accepted after the specified period.
◆ For those who graduated in or before March 2021 → (Individual application)
Please mail the following five items to the Office of Student Services at Keio University Shiba-Kyoritsu Campus in an envelope with "Request for National Examination for Pharmacists Application Documents" written in red ink. Applications will be accepted starting from November 1. They must arrive by Friday, December 2.
A memo stating your year of graduation, department (and student ID number, if known), full name, a daytime phone number, and your preferred method for receiving the application documents (in person at the office or by mail).
One photograph for the examination (6 cm high x 4 cm wide). It must have been taken within six months prior to the application, with your full name written on the back.
A copy of a photo ID (e.g., driver's license, passport).
A 500-yen postal money order for the certificate of graduation issuance fee.
A self-addressed, stamped return envelope, Kakugata 2 size (large enough to hold A4 paper without folding). Clearly write your name and address on the front, write "Kani-kakitome (Simplified Registered Mail)" in red ink, and affix a 460-yen stamp. *The envelope may be folded.
《Please Note》
・The certificate of graduation used for the national examination application is different from a standard certificate and cannot be requested through any procedure other than the one described above.
・After receiving the application documents, you will be applying individually. Please follow the instructions on the Ministry of Health, Labour and Welfare website (especially section 5, Examination Procedures) to complete your application correctly.
・To prevent the spread of COVID-19, access to the Shiba-Kyoritsu Campus is currently restricted to the minimum necessary for educational and research activities. Alumni are not permitted to enter the campus. Please apply by mail.
