About Visiting Each Medical Department
If you wish to visit, please follow the procedures below.
1. Please check the visit dates for the department you wish to visit in the PDF below.
2. Please convert the health checkup status survey form into a PDF and send it as an email attachment to the Postgraduate Medical Education Center.
3. Please fill out the necessary information here and apply at least 14 days before your desired visit date.
* You do not need to attach the results of chest X-rays, viral infection history, or vaccination history.
* Visits are not accepted on Saturdays or hospital holidays.
* Please note that we may not be able to accommodate your request depending on the situation.
